Employee Benefits

Community Connections offers a great benefits package.  Even with the rising costs of health care, Community Connections has maintained its package with little impact on employees. Below is a summary of the benefits for Community Connections employees. Please contact the Payroll and Benefits Manager for a more complete description of benefits and eligibility.

Eligibility

Regular, full time employees and their dependents.

Waiting Period

1st of month after 90 days from date of employment.

Enrollment

You will be contacted prior to your eligibility date to complete paperwork, even if you are electing to decline coverage.

Coverage Options

PPO Plan
HSA Plan
Dental/Vision Plan

Employees pay a portion of the premium based on the coverage selected. This plan allows premiums to be paid on a pretax basis, before federal and state taxes are deducted.

Eligibility

Regular, full time employees who are covered under another medical plan.

Coverage Options

If eligibility criteria are met and the employee waives Community Connections health care coverage, employee can receive a monthly stipend of $300.

Eligibility

You will be contacted prior to your eligibility date to complete paperwork, you must designate beneficiaries which may be changed at any time.

Coverage Options

Employee life and accidental death and dismemberment insurance.

Benefit amounts:
$10,000 Life
$10,000 AD&D

Eligibility

Regular, full time employees. Full time hours must be worked in the pay period of the holiday to be eligible.

Waiting Period

Eligible employees will receive holiday pay upon hire.

Not applicable.

In general, Community Connections observes 11 paid holidays.

Eligibility

Regular, full time employees.

Waiting Period

Work full time each pay period for entire six month probationary period or upon completion of six month probationary period and pass working 1040 hours.

Enrollment

Paid leave is activated by payroll after meeting the coverage waiting period. Leave balances appear on your pay stub once activated.

Coverage Options

4 weeks in first year and an additional 1 day for each year of service up to a max accrual of 6 weeks per year.

Eligibility

All employees.

Waiting Period

Eligible to make contributions upon hire.

After 1 year of service and 1,000 hours, eligible to receive employer match; 100% of the first 6% of an employee’s contribution. You will have a 100% vested and non-forfeitable interest in your account after 5 years of vesting service.

Coverage Options

Roth contributions allow you to contribute on an after-tax basis.
Elective Deferral contributions allow you to contribute on a pretax basis, before federal income taxes are deducted.

Eligibility

All employees.

Waiting Period

Eligible employees can make elective deferrals upon hire.

There is no employer match. You will have a fully vested and non-forfeitable interest in your account immediately.
